Job description

Arnold Medical Centre are looking for an Advanced NursePractitioner to join our innovative and forward thinking Practice to help usmeet our patients needs

They will be able to work autonomously within the scope oftheir experience and qualifications but will have the support and supervisionof the doctors within the practice. This includes appointments within thesurgery as well as visits to patients in their own homes or nursing/residentialhomes where required.

They will provide assessment and care of acutely presentingpatients within the practice.

They will recognise and work within their own competence andprofessional code of conduct as regulated by the NMC. In order to work at thislevel NMC requirements for advanced clinical practice must be met.

Main duties of the job

You will be an Advanced Clinical/Nurse Practitioner and able to manage patients autonomously. These will bewith a minor illness focus across the age range and online triage consultationcover .

The main duties will include assessing and diagnosingpatients presenting with acute issues, either face to face, by telephone or anyother methods as appropriate. You will undertake a variety of duties includingface to face consultations, telephone consultations, and home visits. This willbe for patients across the age range.

You will also assist the nursing team with patients who havepresented with issues beyond those that they feel competent to manage.

About us

We are a team of 3 GPs, 1 Paramedic, 3 Practice Nurses, 1 Healthcare Assistant, and 2 Pharmacy Technicians who are supported by a hard-working and committed team of Reception/Admin staff, performing their roles to the highest of standards whilst prioritising patient care and supporting each other.

The Practice also has a host of additional roles such a Physiotherapist, Podiatrist and Mental Health Practitioner to help us meet the needs of our patients.

Job responsibilities

Assess, examine, diagnose, and develop management plans forpatients presenting with a new problem. Deliver evidence-based care accordingto national and local guidelines.

Prescribe within the scope of knowledge in accordance toboth practice and Lancashire and South Cumbria ICB.

Produce accurate complete records of patient consultationusing accurate codes to ensure easy and accurate information retrieval formonitoring and audit processes.

Assist the practice to obtain the level of QoF pointsrequired by acting on highlighted QoF indicators where appropriate.

Provide information and advice on prescribed orover-the-counter medication on medication regimens, side-effects, andinteractions.

Prioritise health problems and intervene appropriately toassist the patient in complex, urgent or emergency situations, includinginitiation of effective emergency care.

Support patients in the application of the principles ofself-care and healthy living.

Where suitably qualified, to manage the health needs ofwomen presenting for family planning, cervical cytology, or sexual healthconsultation.

Participate in opportunistic vaccination and immunisation ofboth adults and children.

Follow agreed clinical protocols with referral to GPs asappropriate.

If required, to assess and manage the needs of patientspresenting for opportunistic wound care or phlebotomy provision.

Support the nursing team with assessing patients who havepresented with issues beyond those the nurse/HCA feels competent to managewhere it is within the clinical scope of your own competence

General

Take responsibility for your own learning and personaldevelopment to include acting as a positive role model.

Have an awareness of and compliance with all relevantpractice policies/guidelines, such as prescribing, confidentiality, dataprotection, health and safety.

Communicate effectively with patients and carers,recognising the need for alternative methods of communication to overcomecultural and language barriers.

Carry out consultations within the practice face to face, bytelephone or using online or other methods as appropriate.

Use the audit cycle to evaluate the quality of the workwithin the practice and use results to implement improvements where required.

Support and participate in shared learning across thepractice and wider organisation.

Understand and apply legal issues that support theidentification of vulnerable and abused children and adults and be aware ofstatutory child/vulnerable patients health procedures and local guidanceincluding contributing to case conferences or documentation if required.

Maintain and clean equipment as per guidelines that are usedin the course of your duty.

Chaperoning and assisting patients who are being examined byanother clinician on request.

Maintain general tidiness and cleanliness of theconsultation or treatment room that you are working in.

Participation in administrative systems in the Practice

A duty to advise GP(s) of potential problems or errorswithin the range of assigned tasks.

Attend and participate in any Practice meetings whenrequired.

On occasion there may be a requirement to undertake homevisits

Any other delegated duties appropriate to the post
