Job description

We are seeking a skilled and experienced Advanced Nurse Practitioner (ANP) to join our multidisciplinary team at Chiltern House Medical Centre on a part-time basis. This is a fantastic opportunity for an autonomous clinician to play a key role in delivering high-quality care to our patient population. The role involves providing advanced clinical assessments, managing long-term conditions, leading on health promotion and disease prevention initiatives, and contributing to the development of clinical policies and procedures. The ANP will work collaboratively with the wider team to support the delivery of patient-centred care across a range of clinical areas including chronic disease management, well woman and well man clinics, and urgent care.

Main duties of the job

As an Advanced Nurse Practitioner, you will work autonomously within your scope of practice to deliver high-quality, patient-centred care. Key responsibilities include managing acute and chronic conditions, conducting health assessments, running specialist clinics, prescribing medications, and promoting health and disease prevention. You will also contribute to clinical policy development, support safeguarding, and play an active role in clinical audits and service improvement across the practice.

Job responsibilities

Clinical Responsibilities:

In accordance with the practice timetable, the ANP will work autonomously, delivering care within their scope of practice to the patient population. Contributing towards clinical areas such as health promotion, chronic disease management, health prevention, well-women and well man clinics, as well as supporting the management team in reviewing clinical policies and procedures.

  • Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice to patients
  • Implement and evaluate individual specialised treatment plans for chronic disease patients
  • Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects on their health
  • Provide advanced, specialist nursing care to patients as required in accordance with clinical evidence, NICE, and NSF guidelines
  • Provide wound care (ulcer/Doppler etc.) to patients
  • Provide specialist clinics such as ear syringing, contraception, etc.
  • Undertake the collection of pathological specimens
  • Request pathology services as necessary
  • Process and interpret pathology and other test results as required
  • Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required
  • Maintain accurate clinical records in conjunction with extant legislation
  • Ensure read codes are used effectively
  • Maintain chronic disease registers
  • Prioritise health issues and intervene appropriately
  • Support the team in dealing with clinical emergencies
  • Recognise, assess, and refer patients presenting with mental health needs
  • Implement vaccination programmes for adults and children
  • Delegate clinical responsibilities appropriately, ensuring safe practice within scope
  • Support the clinical team with safeguarding matters, in accordance with policies
  • Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately
  • Deliver opportunistic health promotion where appropriate
  • Act as an extended and supplementary prescriber, adhering to guidance
  • Support patients in the use of prescribed and over-the-counter medicines, reviewing annually as required
  • Contribute to practice targets (QOF etc.), complying with guidance
  • Liaise with external services to ensure appropriate patient support, especially vulnerable patients
