Key Responsibilities
Provide advanced clinical assessment, diagnosis,and treatment

Thefollowing are the core responsibilities of the Advanced Nurse Practitioner.There may be, on occasion, a requirement to carry out other tasks; this will bedependent upon factors such as workload and staffing levels:
a. Independentlyassess, diagnose, plan, implement, and evaluate care for patients with complexhealth needs.
b. Prescribemedications and treatments in accordance with professional guidelines and localpolicies.
c. Providepatient education and health promotion to support prevention andself-management of chronic conditions.
d. Performclinical procedures within scope of practice.
e.Develop,implement and embed health promotion and wellbeing programmes
f.Managepatients presenting with a range of acute and chronic medical conditions,providing subject matter expert advice
g.Implementand evaluate individual specialised treatment plans for chronic diseasepatients
h.Identify,manage and support patients at risk of developing long-term conditions,preventing adverse effects to the patients health
i.Provideadvanced, specialist nursing care to patients as required in accordance withclinical based evidence, NICE and the NSF
j.Undertakethe collection of pathological specimens
k.Requestpathology services as necessary
l.Processand interpret pathology and other test results as required
m.Providechronic disease clinics, delivering patient care as necessary, referringpatients to secondary/specialist care as required
n.Maintainaccurate clinical records in conjunction with extant legislation
o.EnsureSNOMED CT codes are used effectively
p.Maintainchronic disease registers
q.Providewell woman clinics
r.Providewell man clinics
s.Prioritisehealth issues and intervene appropriately
t.Supportthe team in dealing with clinical emergencies
u.Recognise,assess and refer patients presenting with mental health needs
v.Bean extended and supplementary prescriber, adhering to extant guidance
w.Supportpatients in the use of their prescribed medicines or over the counter medicines(within own scope of practice), reviewing annually as required
x.Contributeto practice targets (QOF, etc.), complying with local and regional guidance
y.Liaisewith external services/agencies to ensure the patient is supportedappropriately (vulnerable patients, etc.)
z.Delegateclinical responsibilities appropriately (ensuring safe practice and that thetask is within the scope of practice of the individual)
aa. Support the clinicalteam with all safeguarding matters in accordance with local and nationalpolicies
bb. Understand practice andlocal policies for substance abuse and addictive behaviour, referring patientsappropriately
cc. Deliver opportunistichealth promotion where appropriate
