Job description

We are looking to recruit a qualified Advanced Nurse Prescriber to join ourmotivated, friendly and supportive GP Practice in Shere, Surrey. The successfulcandidate will be confident in independently managing acute cases of minorillnesses arising from the clinical triage system. Seeing and reviewingpatients with long term conditions. Supporting the triage and care navigationof patients as well as providing care to patients face to face, over thetelephone and sometimes in their own homes.

Main duties of the job

TheAdvanced Nurse Practitioner (ANP) will work autonomously within the GP Practiceteam to provide advanced clinical care, including assessment, diagnosis,treatment, and management of patients presenting with undifferentiated andundiagnosed conditions. The ANP will also support chronic disease management,health promotion, and the delivery of evidence-based care.

Job responsibilities

Key Responsibilities:

Clinical Care:

  • Provide care to patientswith acute, chronic, and complex conditions.
  • Undertake clinicalassessments, initiate investigations, diagnose, and formulate managementplans.
  • Prescribe medications withinthe scope of professional practice.
  • Refer patients toappropriate secondary or specialist services when needed.
  • Perform clinical procedureswithin the scope of practice (e.g., wound care, joint injections, cervicalsmears, etc.).
  • Lead chronic disease clinics(e.g., asthma, diabetes, hypertension).

Autonomous Practice:

  • Manage a caseload ofpatients independently, with access to GP support as needed.
  • Recognise and act upon redflag symptoms and signs of serious pathology.
  • Maintain accurate, timely,and legible clinical records.
  • Work closely with GPs,nurses, pharmacists, and other multidisciplinary team members.
  • Participate in practicemeetings, clinical audits, and quality improvement initiatives.
  • Support triage and patientflow during high-demand periods.

Development:

  • Contribute to clinicalgovernance, risk management, and safeguarding procedures.
  • Participate in ongoingprofessional development and appraisal processes.

Health Promotion and Education:

  • Provide education and adviceon lifestyle, prevention, and self-care.
  • Participate in patientengagement initiatives and local health campaigns.

Qualifications
  • Qualifications & Requirements:
  • Registered Nurse with valid NMC PIN.
  • Masters Degree in Advanced Clinical Practice or equivalent.
  • Evidence of continued professional development.
  • Minimum 3 years post-registration experience, preferably in primary care.
  • Experience using EMIS Web or other GP clinical systems.
  • Skills in minor illness, minor injury, or womens health.
  • Ability to work well under pressure and adapt to changing demands.
