Main duties of the job
- Triaging E consults as operate a total triage system
- Managing acute and minor illness presentations
- Long-term condition reviews (depending on experience)
- Home visits (if applicable)
- Working closely with GPs, nurses, and our admin team
- Contributing to a collaborative and innovative approach to primary care

Job responsibilities
- Independently assess, diagnose, and manage patients presenting with a wide range of conditions.
- Prescribe medications appropriately in line with current guidelines and within scope of practice.
- Manage acute illnesses, chronic disease reviews, minor injuries, and urgent care presentations.
- Order and interpret diagnostic tests, investigations, and referrals.
- Undertake health promotion, disease prevention, and screening initiatives.
- Provide expert advice and support to patients, carers, and colleagues.
- Work closely with GPs, practice nurses, healthcare assistants, and other team members to ensure coordinated and holistic patient care.

- Masters degree in Advanced Clinical Practice or equivalent
- Independent Prescriber (V300 qualification)
- Proven experience working autonomously in primary care, urgent care, or similar settings
- Excellent clinical reasoning, decision-making, and communication skills
- Commitment to patient-centred care and multidisciplinary working
Desirable
- Experience using EMIS clinical IT systems
- Special interests (e.g. womens health, minor illness, frailty, mental health)
- Teaching, mentoring, or leadership experience
