An exciting opportunity for an Advanced Nurse Practitioner to join a forward-thinking team, responsible for delivering high-quality care and managing a small nursing team. The role involves clinical assessments, prescribing, and coordinating care for patients in a dynamic practice environment. Applicants should have a strong background in advanced nursing with relevant qualifications.
The Advanced Nurse Practitioner will act autonomously within their professional scope of practice providing care for patients face to face, telephone consultations and home visits.
The postholder will operate within their own sphere of competence and agreed acceptable limits of practice to provide expert professional clinical advice to patients, carers and colleagues, ensuring clinical safety and excellence.
The Advanced Nurse Practitioner will need to prioritise the needs of patients accordingly, making any necessary referrals for investigations in the appropriate manner.
The postholder will use skills, knowledge and competencies as a qualified Advanced Nurse Practitioner in order to be responsible and accountable for managing patient caseloads for treatments, referral, admission and discharging patients as appropriate.
The Advanced Nurse Practitioner will manage our team of 3 practice nurses and a HCA, as well as undertake clinical assessment including history taking and physical examination, form a diagnosis and provide a treatment plan, which may include prescribing.

Job responsibilities:
Clinical Nursing Practice
·Delivery of carein line with NSF, NICE guidelines and evidence-based care
·Co-ordinate theplanning and delivery of care ensuring patients and carers/relatives areinvolved.
·Requestdiagnostic investigations and interpret within agreed protocols e.g. x-ray,blood tests, scans, etc.
Care Management
·Communicates confidential and sensitiveinformation to patients, relatives and carers in relation to their condition.
·Provide highly specialised care advicebeyond the scope of normal Nursing practice e.g. care treatment plans, makingreferrals as appropriate
·Contribute to local and nationalpractice targets clinical remit e.g. QOF, prescribing incentive scheme,National benchmarking.
·To monitor and meet care outcomesagainst standards and recommend or initiate changes as necessary.
Health Improvement
·Management of health screeningprogrammes and ensures other team members are aware of health priorities andscreening objectives and programmes.
·Innovation in practice to meet ongoingdemands on the practice’s delivery of care.
·Improve health outcomes for patients byintroducing new developments, and can demonstrate them as reports andpresentations where appropriate.
·Supports practitioners in reporting thepractice population to promote health improvement and disease prevention.
·Makes referrals to appropriate agencies(statutory and voluntary) where necessary

Administrationand professional responsibilities
·Produceaccurate, contemporaneous and complete records of patient consultation,consistent with legislation, policies and procedures
Training and personal development
·Assesseffectiveness of care delivery through self-reflection and peer review, as wellas benchmarking and formal evaluation.
·Contributeto the identification and assessment of learning needs of staff and otherprofessionals and assist in planning effective programmes of education.
Manage,supervise, co-ordinate and guide the nursing team
