An established industry player is seeking an Advanced Nurse Practitioner to lead clinical initiatives within a dedicated Home First Team. The Advanced Nurse Practitioner is responsible for providing senior clinical leadership and advice within the Home First Team, which offers crisis and intensive support to patients. The role requires the post holder to provide managerial input, clinical leadership, and day-to-day coordination of clinical practice to support effective care delivery across the Urgent Care Pathway.
You will collaborate with key partner organisations, service users, carers, clinicians, and other practitioners within the multidisciplinary team to deliver services. This includes providing a 7-day-a-week, 24-hour service, 365 days a year, working shifts as appropriate and operationally required. Participation in projects to review and develop service provision is also a requirement.
You will lead, alongside other senior colleagues, the ongoing development of a high-quality service grounded in evidence-based practice, care pathways, NICE guidance, and DH guidance. You will lead service development and policy in conjunction with other Advanced Practitioners, senior clinicians, the Clinical Manager, and the Operational Services Manager.
You will also oversee the day-to-day supervision and management of lower banded staff within your team, as required by your manager, in accordance with Trust HR processes.

You will be responsible for proactive assessment, monitoring, and management of patients, ensuring care is current, evidence-based, and aligned with standards. You will provide clinical leadership, evaluate and monitor the service, and offer specialist assessments as needed.
This role requires a DBS check due to the Rehabilitation of Offenders Act (Exceptions Order) 1975.
