We have an exciting opportunity for Advanced Nurse Practitioners to join our team on a full time basis at the Staunton Group Practice.

The post is for an experienced Advanced Nurse Practitioner who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment including prescribing and evaluation of care. They will demonstrate skills, for patients within the general practice.

The post holder will demonstrate critical thinking in the clinical decision-making process. They will work collaboratively with the general practice team to meet the needs of the patients, supporting the delivery of policy and procedures, and providing nurse leadership as required. To work at this level NMC requirement for advanced practice must be met.

  • Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis.
  • Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
  • Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan.
  • Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice.
  • Work with patients to support compliance with and adherence to prescribed treatments Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions.
  • Provide management support and leadership to the practice nurse team, including appraisals, plus training and development

Job responsibilities
  • To be responsible for the delivery of practice nursing services, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the entitled patient population.
  • To be responsible for a number of clinical areas such as health promotion, chronic disease management, health prevention, well women and well man clinics, as well as supporting the management team in the reviewing of clinical policy and procedure. In order to work at this level the post holder must be a registered general nurse (currently registered the Nursing & Midwifery Council), have a recognised NP qualification (minimum of degree level or equivalent) and be an independent Nurse Prescriber.
  • To act as an autonomous practitioner working independently and in conjunction with other health care professionals to assess, diagnose and treat the conditions of patients attending within primary care initiating direct referrals as appropriate.
  • Provide expert professional advice to patients, carers and colleagues, with a holistic approach, and ensure the maintenance of clinical excellence.
  • To develop new and innovative concepts, models, methods and practices to deliver new and improved primary care services to meet the needs of the practice and PCT population.
  • To provide education and training to other staff and students.
  • Will act within their own boundaries of competence and agreed acceptable limits of practice to work as an autonomous practitioner
  • Provide expert professional clinical advice and care to patients face to face, telephone consultations, and home visits ensuring clinical safety and excellence.
  • The post holder will use skills, knowledge and competencies as a qualified Advanced Nurse Practitioner including prescribing and take responsiblilty and accountability for managing caseloads for treatments and referrals.
