Job description

James Fisher Medical Centre is seeking 1 or 2 experienced Advanced Nurse or Clinical Practitioners to join our friendly GP practice in Muscliffe, Bournemouth. Newly qualified ACPs will also be considered, but we are not accepting applications from Physician Assistants at this time. We are a well-established and successful practice with 12,500 patients and are part of the Bournemouth Central Primary Care Network.

Our team includes 5 GP Partners and 4 Salaried GPs, Practice Nurses, Phlebotomists, and Health Care Assistants. We currently have 2 Advanced Practitioners who will be moving on at the end of August, so we are open to recruiting for 2 roles. Our clinical team works alongside our excellent reception and administration teams, as well as Central Bournemouth Primary Care Network staff, some of whom hold sessions within the practice.

The positions are advertised as full-time (37.5 hours per week, Monday to Friday), but we will also consider part-time hours (minimum 2.5 days per week).

Main duties of the job

We're seeking 1 or 2 skilled Advanced Practitioners with strong experience in a primary care setting. The role involves supporting Duty GPs with managing acute same-day patients, including children. Patients are triaged in advance by our Duty GP team. Candidates should have experience managing patients with a wide range of acute and chronic medical conditions. The successful candidates will support and work collaboratively with the GP team to ensure patients are seen by the most appropriate clinician.

About us

James Fisher Medical Centre has a diverse population, including many families and a small number of care homes.

We use Systmone for our patient records, and all GP appointment requests are triaged by the duty doctor team to ensure patients are seen by the most appropriate clinician. Our Advanced Practitioners work alongside the duty doctors, independently managing triaged patients with close access to the Duty GP team. We have large, purpose-built premises in a beautiful area of the south coast, with nearby riverside walks accessible during lunch breaks.

Job responsibilities

The core responsibilities of the Advanced Clinical Practitioner include, but are not limited to:

  • Practising in accordance with professional conduct and within scope of practice, being responsible and accountable for decisions and actions.
  • Demonstrating understanding of their responsibilities and autonomy, recognizing limits of competence.
  • Acting on professional judgement and seeking help when necessary.
  • Using assessment methods to work in partnership with individuals, families, and carers.
  • Supporting patients in making decisions, planning care, and making positive changes.
  • Using clinical reasoning and decision-making skills.
  • Initiating, evaluating, and modifying interventions, including prescribing medicines and therapies.
  • Managing risks professionally.
  • Managing patients with acute and chronic conditions and providing expert advice.
  • Supporting patients at risk of long-term conditions and preventing adverse health effects.
  • Being a professional role model.
  • Engaging in peer review and continuous improvement.
  • Seeking feedback and involving individuals and communities in service improvements.
  • Addressing personal learning needs and developing professionally.
  • Maintaining accurate clinical records and using SNOMED CT codes effectively.
  • Appraising research and audit outcomes to inform practice.
  • Developing governance systems and contributing to practice targets.
  • Chaperoning patients when necessary.
  • Supporting the team in clinical emergencies and safeguarding matters.
  • Understanding policies on substance abuse and delivering health promotion.

Additionally, the ACP may support infection prevention and control, supervise medicines storage, mentor staff, participate in initiatives, develop specialist interests, review practices, participate in incident reviews, support service development, and develop protocols.
