To work as an autonomous practitioner, workingas part of the practice multidisciplinary team, delivering care within theirscope of practice to the patient population. Manage patients presenting with a range of acute andchronic medical conditions, providing subject matter expert advice to patients
The Advanced Nurse/Clinical Practitioner will be responsible forseveral clinical areas such as health promotion, minor illness, as well assupporting the management team in the reviewing of clinical policies andprocedures.
The post holderworks at the level of Advanced Nurse/Clinical Practice and has developed andcan evidence the underpinning competencies applicable to the specialty orsubject area, i.e., the knowledge, skills and behaviours appropriate to the healthcareprofessionals (HCP) setting and job role.
The roleprovidesan opportunity for patients to avoid the need to be referred to a GP, therebyenabling them to receive timely care and negate unnecessary delay in receivingtreatment.
An ANP/ACPin primary care can develop close, long-term relationships with their patientsand work in partnership with them to achieve optimum health. They are autonomous in making decisions basedon assessment, diagnosis and interpretation of test results. They can independently prescribe appropriatemedication, evaluate or refer to other specialists if necessary.
The TardisSurgery is one of 7 practices within the Moorlands Rural PCN.
We currentlyhave 5,900 patients on the practice list; the surgery is set in a semi-ruraltown,
We provideservices to two nursing homes and the Moorlands Neurological centre providingweekly visits to the homes and monthly MDTs. We are also a dispensing practiceserving our patients who live more than a mile away from a community Pharmacy.
We are a veryfriendly team and try to support our staff to continue to develop.
Our staffingconsists of:
2 GP Partners,ANP, Practice Nurse, HCSW, Dispensers, Clinical coder, Clinical Administrator,Reception/Admin team and a management team.
We also havesupport from our Moorland Rural PCN ARRS team which consists of:
FCP, MHP, STRworker, Pharmacy team, Care coordinator, HWBC, counsellor, and socialprescribers.
We engageclosely with our local community getting involved with various activities e.g.supporting the local High school with careers days, primary school wellbeingmorning etc.
We have a very committedPPG who work alongside us to ensure we are delivering high quality personcentred care.

Induction

Theeffective use of training and development is fundamental to ensuring that allstaff are equipped with the appropriate skills, knowledge, attitude, andcompetences to perform their role. All staff will be required to partake in andcomplete mandatory training as directed by Moorlands Rural PCN It is anexpectation for this post holder to assess their own learning needs andundertake learning as appropriate
Thepost holder will provide an educational role to patients, carers, families andcolleagues in an environment that facilitates learning.
Allstaff are to recognise the significance of collaborative working, understandtheir own role and scope, and identify how this may develop over time. Staffare to prioritise their own workload and ensure effective time-managementstrategies are embedded within the culture of the team.
Managinginformation
Allstaff should use technology and appropriate software as an aid to management inthe planning, implementation and monitoring of care, and presenting andcommunicating information.
Datashould be reviewed and processed using accurate SNOMED/read codes to ensureeasy and accurate information retrieval for monitoring and audit processes.
Servicedelivery
Staffwill be given detailed information during the induction process regardingpolicy and procedure.
Thepost holder must adhere to the information contained within the organisationspolicies and regional directives, ensuring protocols are always adhered to.
Security
Thesecurity of the organisation is the responsibility of all personnel. The postholder must ensure they always remain vigilant and report any suspiciousactivity immediately to their line manager.
Underno circumstances are staff to share the codes for the door locks with anyone,and they are to ensure that restricted areas remain effectively secured.Likewise, password controls are to be maintained and passwords are not to beshared.
Professionalconduct
Allstaff are required to dress appropriately for their role.
Leave
Allpersonnel are entitled to take leave. Line managers are to ensure all theirstaff are afforded the opportunity to take a minimum number of days leave eachyear and are encouraged to take all their leave entitlement.
Publicholidays will be calculated on a pro-rated basis dependent on the number ofhours worked.
