In OOH, we assess and treat patients who require urgent unscheduled intervention suitable for treatment by general practice clinicians when the GP surgeries are closed. • Undertake home visits to patients requiring assessment or treatment in their own residence, offering a personalized and compassionate approach.
  • Manage telephone triage, providing expert clinical advice, risk assessment, and directing patients to appropriate care pathways.
  • Work autonomously while collaborating closely with a multidisciplinary team to ensure seamless, patient-centred care.
  • Maintain accurate clinical records and adhere to Practice Plus Group policies and NHS guidelines to uphold high standards of care.
About us
  • Registered Nurse with MSc in Advanced Clinical Practice or equivalent
