Job summary

West Heath Surgery is seeking an Advanced Medical Practitioner to deliver high-quality clinical care to residents of designated care homes. You will work as part of the practice multidisciplinary team, collaborating with GPs, advanced practitioners, nurses, pharmacists, care coordinators and care-home staff to provide proactive, personalised care and medicines optimisation.

Responsibilities

• Provide high-quality, safe and person-centred clinical care to care-home residents.
• Collaborate with GPs, advanced practitioners, nurses, pharmacists, community staff and care-home teams.
• Contribute to proactive, personalised and coordinated healthcare and medicines optimisation.
• Support delivery of NHS England Network Contract DES and Enhanced Health in Care Homes requirements.
• Escalate appropriately when GP, consultant or other input is required.
