HealthHero is seeking an independent Prescribing Clinician to support Care Coordination and Integrated Urgent Care across Bath, Swindon and Wiltshire on a permanent basis. Flexible part-time or full-time arrangements are available on a fixed rota pattern.
The role blends clinical assessment with system navigation to improve patient outcomes, reduce hospital admissions, and optimise care pathways. Shifts include triage at Head Office, face-to-face at Primary Care Centres, and mobile home visits.
We have an exciting opportunity for an independent, autonomous Prescribing Clinician to play a key role in our Care Coordination and Integrated Urgent Care services across the Bath, Swindon and Wiltshire locality on a permanent basis. We can support both part-time and full-time working arrangements on a fixed rota pattern basis.
This role offers a unique blend of clinical assessment and system navigation, supporting patients to access the right care, in the right place, at the right time, helping to reduce unnecessary hospital admissions and improve patient outcomes.
You will work across our Care Coordination service in hours and to support our Urgent Care service out of hours. Our Care Coordination shifts are based at our Chippenham Head Office and our out of hours sessions will be based at one of our Primary Care Centres.
As a Prescribing Clinician within our Care Coordination function, you will:

Please note that remote senior clinical support is always available during your shift.
You will be a registered nurse, paramedic or allied health professional with a non-medical prescribing qualification, able to work confidently within a broad scope of practice.
Our Integrated Urgent Care (IUC) is a service offered to patients who are registered with a General Practitioner within the Bath and North East Somerset, Swindon, and Wiltshire Integrated Care System.
