HealthHero is seeking a Prescribing Clinician to join our Care Coordination function on a part-time, permanent basis with regular weekend shifts from our Chippenham Head Office. You will support urgent care out-of-hours across Bath, Swindon and Wiltshire, combining remote consultations, face-to-face care and home visits to optimise patient pathways.
You will be a registered nurse, paramedic or allied health professional with a non-medical prescribing qualification, and have at least 2 years
We have an exciting opportunity for an independent, autonomous Prescribing Clinician to play a key role in our Care Coordination and Integrated Urgent Care services across the Bath, Swindon and Wiltshire locality. We are seeking a motivated individual to join our team on a part-time, permanent basis, working regular weekend shifts.
You will work across our Care Coordination service in hours and to support our Urgent Care service out of hours. Our Care Coordination shifts are based at our Chippenham Head Office and our out of hours sessions will be based at one of our Primary Care Centres. This role offers a unique blend of clinical assessment and system navigation, supporting patients to access the right care, in the right place, at the right time, helping to reduce unnecessary hospital admissions and improve patient outcomes.

You will be a registered nurse, paramedic or allied health professional with a non-medical prescribing qualification, able to work confidently within a broad scope of practice.
