We are looking for a motivated, dynamic, and flexible band 7 Occupational Therapist for a fixed term position within East Cheshire Trust Transfer of Care Hub (TOCH). The Home First Occupational Therapist role is funded as part of the discharge investment scheme supporting the implementation of the Home First model across Cheshire East, due to its proven success supporting people to return home and reducing care hours the scheme has been extended until March 26.
The main objective of the Occupational Therapist is to work in collaboration with inpatient and community therapies, Care4CE Reablement service and Right at Home Care provider to support discharge from hospital or a community bed.

Whilst COVID-19 vaccination is not a condition of employment, we do encourage our staff to get vaccinated. We will be checking the vaccination status of all new starters as part of ongoing monitoring.
