Role Summary

We are looking for a highly experienced Advanced Clinical Specialist to join our Acute Frailty Service for an 18-month fixed-term or secondment opportunity. You will work across our same day emergency care service (SDEC), Emergency Department and Medical Assessment Unit. You will need advanced comprehensive geriatric assessment and holistic management skills and will work closely with our Pathfinder and Acute Hospital at Home teams.

This is primarily a clinical role, although you will also have some line-management responsibilities. You will need a comprehensive understanding of frailty and be able to work with a holistic mindset across traditional professional boundaries. You will also have opportunities to deliver frailty education to colleagues and other teams, and to help drive service development that improves the care and management of older people living with frailty.

Working closely with acute and community colleagues, you will provide a vital link between services and support seamless transitions from hospital to home or another receiving destination. The role is challenging and time-sensitive, helping to support patient flow through the acute hospital, but it is also varied and rewarding.

We are seeking a forward-thinking, experienced, motivated and creative individual to join us. The successful candidate will be able to work effectively as part of a team, remain calm under pressure and contribute positively in a fast-paced environment.

The successful applicant will receive training and support, including a robust induction tailored to their needs. We are a friendly, flexible and supportive team who face challenges together and support one another through both the good and difficult times.

Monday to Friday; hours are to be discussed.

Working Pattern:Monday - Friday 08:00 - 18:00 - 28.5 hours
