Job summary

Qualifications
- NMC/ GPhC / HCPC registered professional
- MSc in Advanced Clinical Practice or equivalent
- Evidence of an ACP/Frailty portfolio
- Teaching/Assessment/Mentorship qualification (supervising and assessing)
- Level 7 Advanced physical assessment module
- ALS
- Frailty related Level 7 studies
- Leadership qualification or relevant leadership experience
- Research in practice qualifications or equivalent
Responsibilities
- Deliver advanced clinical care to acutely unwell patients in the community with comprehensive assessment and management planning.
- Lead service improvement initiatives and contribute to governance, education and workforce development.
- Provide clinical supervision and teaching, participate in audits, research and quality improvement.
- Promote evidence-based practice and contribute to resource and risk management to ensure safe clinical practice.
