Job summary

Derby Road Practice in the Suffolk Primary Care partnership is seeking an Advanced Clinical Practitioner to join its urgent on-the-day care service. The role focuses on assessment, diagnosis and management of acute and chronic conditions, with prescribing under PGD or independent routes for adults and children.

Part-time hours (20–30 per week, Wed–Fri) with flexibility for training and leave. Requires an independent prescriber qualification and registration as a nurse; NHS pension benefits,

Qualifications

• Independent prescriber qualification
• Basic Life Support resuscitation training as a minimum
• Registered Nurse status or equivalent

Responsibilities

• Assess patients with undifferentiated conditions and plan care
• Diagnose and manage acute and chronic conditions within a management plan
• Initiate emergency care when required
• Review medications under PGD or independent prescribing
• Advise on prescribed and non-prescribed medicines and interactions
• Maintain accurate patient notes and records
• Work with senior clinicians to inform decisions on patient care
• Adhere to NICE and local guidelines and participate in audits and research
