Job summary

Practice Plus IUC is seeking an Advanced Nurse Practitioner (ANP) or Advanced Clinical Practitioner (ACP) to join the Suffolk and North East Essex UCCH in Ipswich. You will provide remote consultations and manage referrals, coordinating care with urgent care, community and mental health teams.

You will hold an ANP/ACP level qualification with independent prescribing, and contribute to service development, patient safety and professional standards within a multidisciplinary team.

Qualifications

• Master's level ANP/ACP education.
• Non-medical prescribing qualification.
• Six months or more experience in primary care including home visits, triage and face-to-face consultations.
• Independent prescribing experience in urgent, primary or community care.

Responsibilities

• Assess and manage patients via telephone and face-to-face consultations when needed.
• Work autonomously within scope, coordinating referrals across primary, community and secondary care.
• Contribute to service development, governance, safeguarding and quality assurance.
