Primary Care 24 (PC24) seeks Advanced Clinical Practitioners to join our bank workforce across Merseyside GP practices. You will work with autonomy, delivering high-quality, patient-centred care, including assessments, diagnoses and treatment planning.

Salary: £33.28 per hour

Location: Primary Care 24 Hub in Sefton and GP practices in Liverpool & Sefton.
Shifts: Bank positions, operational hours are Monday to Friday 8am - 6:30pm
Undertake comprehensive health assessments for adults and paediatric patients, applying advanced clinical judgement and holistic care planning.
Assess, diagnose, plan, implement and evaluate treatment and interventions for undifferentiated and undiagnosed conditions.
Perform clinical examinations and interpret findings from both physiological and psychological perspectives.
Manage a clinical caseload with a high degree of autonomy, making safe and timely decisions within agreed governance frameworks.
Contribute to a culture of patient safety, quality improvement and effective multidisciplinary working.
Maintain professional competence and development in line with regulatory requirements, contributing positively as a senior clinician within the practice team.
The Primary Care role is designed for ACPs who feel they are confident working autonomously in a GP practice setting. The post holder will have access to GP clinical supervision and clear escalation pathways, with support focused on safe decision-making, governance and clinical case discussion. PC24 promotes a pragmatic and inclusive working environment, where clinicians are encouraged to seek advice when required and professional accountability is shared within established systems of care.
£33.18 per hour

You will practice with autonomy, using advanced clinical skills to assess, diagnose, treat and manage patients both face-to-face and via telephone consultations. This role offers variety, professional challenge and the opportunity to work within a pragmatic and forward-thinking primary care environment.
