This is an excitingopportunity to be at the forefront of the delivery of our Ageing Well model ofcare - Team up Derbyshire. The role transcendsthe traditional health and social care organisational boundaries. It will involve true MDT working, buildingrelationships with GP practices, Community Core and Rapid Response Nursing andTherapy services, Falls services and Adult Social Care, to help clinicallydeliver the service on a day-to-day basis.

This is an evolving service,so you will have the opportunity to contribute to the development of Team Upand the Home Visiting Service if you wish to.

Main duties of the job

South Hardwick PCN are seeking torecruit an Advanced Clinical Practitioner or Trainee Advanced ClinicalPractitioner to work alongside a multi-disciplinary team providing care tohousebound patients and patients in care or residential facilities, either inperson or remotely.

The aim for the service isto ultimately provide a holistic approach to acute on the day/rapid responseservices, enhanced health in care homes and enhanced proactive care for olderpeople with frailty and patients with multi-faceted health problems.
