PELC is seeking an enthusiastic AdvancedClinical Practitioner or Advanced Nurse Practitioner with independent prescribingto work with us across our sites. You will deliver face-to-face services,enabling patients to obtain health information from the appropriate health careprofessional. You will be providing safe and effective health assessments andtreatment of patients within the establishment
You will be accountable for your practiceincluding the assessment, diagnosis, treatment, and discharge or referral ofpatients, independent of medical intervention to provide high-quality supportfor professional practice within the organization.
The post holder will be an experienced caring nurse/practitionerwho can demonstrate highly developed clinical skills and be able to demonstratecritical thinking whilst working within their professional boundaries. You willwork within a clinical team consisting of GPs, UCPs,Nurse Streamers and HCA to deliver excellent care serviceswithin our UTC. You will be able todemonstrate safe, competent clinical decision-making and deliver expert care,including assessment, diagnostic skills, treatment and evaluation. You willcommunicate at all levels across boundaries and work closely with widerclinical team members. The post holder will also work collaboratively with themultidisciplinary team to meet the needs of patients, supporting the deliveryof policy and procedures.
You will be an integral member of the clinical team,completing streaming, independently assessing patients and prescribing clinicalintervention. You will also be completing administrative tasks (prescriptions,referrals, pathology results).
As an ANP/ACP, you will be supported by, and work with, a diverse multi-skilledprofessional team of clinical and non-clinical friendly staff, providing high quality, person-centred,holistic clinical care to patients. You will be accountable for your practice including the assessment, diagnosis, treatment, and discharge or referral of patients, independent of medical intervention to provide high-quality support for professional practice within the organization

Main duties of the job
Utilising yourskills in managing patients within the urgent treatment care setting. You will actas an autonomous advanced practitioner who is able to work independently and inconjunction with other members of the clinical team. As an AdvancedNurse Practitioner/advanced clinical practitioner, you will be supported by,and work with, a diverse multi-skilled professional team of clinical andnon-clinical friendly staff, providinghigh quality, person-centred, holistic clinical care to patients.
Clinical practice
Assess, diagnose, plan, implement and evaluate treatment/interventionsand care for patients presenting with an undifferentiated diagnosis.
Clinically examine and assess patient needs from a physiological andpsychological perspective, and plan clinical care accordingly.
Assess, diagnose, plan, implement and evaluate interventions/treatmentsfor patients with complex needs.
Proactively identify, diagnoseand manage treatment plans for patients at risk of developing a long-termcondition (as appropriate).
Diagnose and manage both acute and chronic conditions, integrating bothdrug- and non-drug-based treatment methods into a management plan.
Prescribe and review medication for therapeutic effectiveness,appropriate to patient needs and in accordance with evidence-based practice andnational and practice protocols, and within scope of practice.
Work with patients in order to support compliance and adherence toprescribed treatments. Provide information and advice on prescribed orover-the-counter medication on medication regimens, side-effects andinteractions.
Prioritise health problems and intervene appropriately to assist thepatient in complex, urgent or emergency situations, including initiation ofeffective emergency care.
Support patients to adopt health promotion strategies that promotehealthy lifestyles, and apply principles of self-care.
Assess, identify and refer patients presenting with mental health needsin accordance with the NSF for Mental Health.
Promote and deliver evidence-based care for patients presenting withaural conditions.
Meet the needs of patients presenting for opportunistic wound care.
Communication
Utilise and demonstrate sensitive communication styles, to ensurepatients are fully informed and consent to treatment.
Communicate effectively with patients and carers, recognising the needfor alternative methods of communication to overcome different levels of understanding,cultural background and preferred ways of communicating.
Anticipate barriers to communication and take action to improvecommunication.
Maintain effective communication within the practice environment andwith external stakeholders.
Act as an advocate for patients and colleagues.
