Overview
PELC is seeking an enthusiastic Advanced Clinical Practitioner or Advanced Nurse Practitioner with independent prescribing to work with us across our sites. You will deliver face-to-face services, enabling patients to obtain health information from the appropriate health care professional. You will be providing safe and effective health assessments and treatment of patients within the establishment. You will be accountable for your practice including the assessment, diagnosis, treatment, and discharge or referral of patients, independent of medical intervention to provide high-quality support for professional practice within the organization.
Main duties
The post holder will be an experienced caring nurse/practitioner who can demonstrate highly developed clinical skills and be able to demonstrate critical thinking whilst working within their professional boundaries. You will work within a clinical team consisting of GPs, UCPs, Nurse Streamers and HCA to deliver excellent care services within our UTC. You will be able to demonstrate safe, competent clinical decision-making and deliver expert care, including assessment, diagnostic skills, treatment and evaluation. You will communicate at all levels across boundaries and work closely with wider clinical team members. The post holder will also work collaboratively with the multidisciplinary team to meet the needs of patients, supporting the delivery of policy and procedures. You will be an integral member of the clinical team, completing streaming, independently assessing patients and prescribing clinical intervention. You will also be completing administrative tasks (prescriptions, referrals, pathology results). As an ANP/ACP, you will be supported by, and work with, a diverse multi-skilled professional team of clinical and non-clinical friendly staff, providing high quality, person-centred, holistic clinical care to patients.
Responsibilities also include:
- Be an autonomous advanced practitioner who can work independently and in conjunction with other members of the clinical team in urgent treatment care settings.
- Contribute to the delivery of safe and effective care within our UTC and comply with policy and procedures.
- Collaborate with GPs, Urgent Care Practitioners, Nurse Streamers and Health Care Assistants to deliver care.
- Complete streaming, independently assess patients, and prescribe clinical interventions.
- Perform administrative tasks (prescriptions, referrals, pathology results).
Clinical practice
- Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis.
- Clinically examine and assess patient needs from physiological and psychological perspectives, and plan care accordingly.
- Assess, diagnose and manage patients with complex needs; identify and manage long-term conditions where appropriate.
- Diagnose and manage acute and chronic conditions, integrating drug- and non-drug-based treatments; prescribe and review medications within scope of practice and evidence-based guidelines.
- Support adherence to prescribed treatments; provide information on regimens, side-effects and interactions.
- Prioritise health problems and intervene in complex, urgent, or emergency situations, including initiation of emergency care.
- Promote health promotion and self-care; assess and refer patients presenting with mental health needs per NSF for Mental Health.
- Promote evidence-based care for aural conditions and opportunistic wound care as appropriate.
Communication
- Use sensitive communication styles to ensure informed consent.
- Communicate effectively with patients and carers, adapting for different levels of understanding, culture and preferences.
- Anticipate communication barriers and take steps to overcome them; maintain communication within the practice and with external stakeholders; act as an advocate for patients and colleagues.
Delivering a quality service
- Work within own competence and professional codes of conduct (e.g., NMC and HCPC).
- Maintain accurate records of patient consultations in line with legislation and policies.
- Prioritise and manage own workload to maintain quality of care.
- Deliver care according to NSF, NICE guidelines and best practice; evaluate care delivery through self and peer review and formal evaluation.
- Contribute to shared learning across the practice and wider organisation; understand legal issues related to safeguarding vulnerable children and adults.
Professional practice
- Provide professional practice support for clinical development of students; promote the vision, values and goals of PELC.
- Demonstrate fairness, respect and professional behaviours; lead and influence others accordingly.
- Support service improvement plans and patient safety governance to improve service delivery.
Autonomy/Scope within the role
- Assume clinical accountability for patients referred into non-medically led services.
- Practice autonomously without direct supervision in line with professional codes and local policies; recognise limits of own competence and undertake only appropriate activities.
- Use professional judgment to address novel problems; apply evidence-based practice and contribute to clinical protocols and care pathways.
- Collaborate with professionals to develop integrated care pathways; influence the culture and environment to humanise patient care; monitor and lead improvements in clinical care through audits, teaching and leadership.
Qualifications
Essential
- Registered with NMC or HCPC with advanced clinical practice/Advanced Nurse Practitioner qualification
- Nurse-led/HCPC management of minor illness, or minor ailments and injuries
- Evidence of post-registration study at undergraduate level (Level 6 or 7 in minor illness)
Desirable
- Experience of clinical and educational programmes
- Relevant Education Qualification (PG Cert Clinical Education or equivalent)
- Master degree in advanced clinical practice

- Strong interpersonal and mentoring skills; experience with older people; partnership working and stakeholder engagement
Desirable
- Participation in clinical supervision or Professional Nurse Advocate programmes
- HR policy understanding; programme design and workforce development initiatives; leading quality improvement initiatives
