Job summary

Sheffield Health Partnership University NHS Foundation Trust invites applications for an Advanced Clinical Practitioner in the Memory Service. The role focuses on autonomous clinical intervention, assessment, diagnosis and management of cognitive impairment within a multidisciplinary Older Adult Services team.

The post involves prescribing within scope, leading service development, and training colleagues while ensuring evidence-based practice across the MDT.

Qualifications

• MSc in Advanced Clinical Practice is required.
• Registration with NMC or HCPC is required.
• Non-medical prescribing is required.
• Ability to work within patient group directions (PGDs).
• Minimum of 5 years post-registration clinical experience.
• Experience in mental health, particularly older adults.

Responsibilities

• Deliver autonomous expert clinical intervention and care planning for patients with cognitive impairment.
• Assess, diagnose and manage patients presenting with cognitive impairment.
• Provide independent review of treatment plans.
• Advise and support B6 Assessors in the team.
• Provide clinical leadership, advice and training to the team with the team manager.
• Ensure delivery of evidence-based practice within the service.
• Apply the four pillars of advanced practice to maintain and improve service quality.
• Prescribe within an agreed scope.
• Work within the multidisciplinary team, including at MDT meetings.
