NHS seeks an Advanced Clinical Practitioner for the Humber Virtual Frailty Ward to provide holistic care for vulnerable patients across Scarborough. You will join a multidisciplinary team, assisting in patient assessments, managing risks, and contributing to clinical management. This part-time role involves working remotely with general practitioners and other professionals to enhance patient care.
Hours: 4 sessions- 2 sessions on a Wednesday and 2 sessions on a Friday- to be worked by one person or could be split between two people.
As a skilled Advanced ClinicalPractitioner with the Humber Virtual Frailty Ward, you will be deliveringholistic care to frail and vulnerable patients across Scarborough, Whitby andRyedale.
Based at the teams Community Hub inScarborough, you will work as member of a multidisciplinary team that includesAdvanced Complex Care Managers, a Senior Physiotherapist, a Specialist ClinicalPharmacist and Nursing Associates. Theteam is supported daily by a Frailty General Practitioner working remotely inour innovative York Community Frailty Hub, and is able to draw on the expertiseand resources of other Humber Community teams such as District Nurses and HeartFailure Specialist Nurses.
