Job summary

Gordon Street Medical Centre in Ashton-under-Lyne is seeking an Advanced Clinical Practitioner to assess and holistically examine patients with both minor and complex needs, performing history taking, examinations and investigations as needed to confirm diagnosis.

You will plan and implement an effective management plan, ensuring safe discharge or referral, triage referrals, and provide clear medication information while upholding high standards of patient safety and governance.

Qualifications

• Experience in clinical assessment and management of patients with minor and complex needs.
• Ability to triage referrals and communicate effectively with patients and colleagues.
• Understanding of NICE guidelines and evidence-based care.

Responsibilities

• Assess and holistically examine patients with minor and complex needs, using history taking, physical examination and investigations as appropriate.
• Plan, implement and evaluate an effective management plan, ensuring safe discharge/referral or transfer as needed.
• Provide information and advice on medications, regimens, side-effects and interactions.
• Triage telephone referrals and calls from external clinicians, patients and others, routing to appropriate services.
• Maintain accurate contemporaneous records of patient consultations in line with legislation and policies.
• Deliver care in line with NSF, NICE guidelines and quality governance processes, and participate in ongoing professional development.
