Job summary

Torbay and South Devon NHS Foundation Trust is seeking an Advanced Clinical Practitioner to deliver comprehensive frailty care within the SDEC pathway and hospital-at-home services. You will work autonomously across clinical leadership, education, and research domains to drive safe, patient-centred care for older adults.

You will possess a Master's in Advanced Clinical Practice or NHSE portfolio route, with non-medical prescribing qualification and registered status.

Qualifications

• Registration with a statutory profession body (NMC/HCPC).
• Master's degree in advanced practice or HE portfolio route.
• Independent non-medical prescribing qualification.

Responsibilities

• Clinical governance, risk management and safety oversight across the MDT.
• Lead OR contribute to audit, evaluation and quality improvement projects.
• Provide expert clinical leadership and supervision within frailty and urgent care services.
• Deliver and develop ACP/advanced skills capability within the workforce.
