Job summary

The Bournemouth East Collaborative Primary Care Network has a new and exciting opportunity for anACPto join the BEC PCN Team. This is a role that combines work with our Enhanced Care Team supporting our housebound patients with acute visits, and work in our Treatment Centre which is largely on the day acute clinics. The hours will be both in-hours and out of hours.

Sessions to be discussed at interview, this will include Tuesday & Thursday evenings up to 8pm.

This role will require anEnhanced DBS Clearance. Should you have this certificate as part of the DBS subscription service, we would be happy to accept this providing the original is shown. Otherwise, you will be expected to complete a new application.

Main duties of the job

The ACP will be expected to provide assessment, diagnosis, and treatment to acute home visits and running acute on the day clinics F2F in our PCN Treatment Centre.

If appropriate, prescribe and refer patients to appropriate care providers, within locally agreed PCN healthcare pathways and complete 2 week waits if required.

Pharmacy team comprising of Clinical Pharmacists, Pharmacy Technicians and Care Co-ordinator.

Mental Health practitioners.

Health & Wellbeing Coaches and Social Prescribers within Help & Care team.

Autonomously manage own visiting list of frailty and housebound patients across the age continuum to provide whole episode of healthcare by seeing, assessing, diagnosing, treating and discharging or referring patient.

Autonomously manage own appointment list of patients across the age continuum to provide whole episode of healthcare by seeing, assessing, diagnosing, treating and discharging or referring patients attending the Treatment Centre across a wide range of undifferentiated and undiagnosed conditions including minor injuries and minor ailments.

Able to use an advanced level of professional accountability, autonomy and judgement for seeing treating and assessing patients who present with a broad range of situations and conditions.

You will have an advanced practice qualification and be a non-medical prescriber.

Plan and manage complete episodes of care, working in partnership with other members of the multi-disciplinary PCN team, delegating and referring as appropriate, to optimise health outcomes for patients.

Work autonomously, clinically examining and assessing patients and initiating any appropriate action whilst taking responsibility for managing practice whilst using an advanced level of professional accountability, autonomy and judgment for highly complex decision making across a broad range of situations.
