We are looking to recruit a full time Advanced Clinical Practitioner for Specialist Palliative Care.

This exciting new role will clinically lead our integrated specialist palliative care team.

Working within the four pillars of advanced practice we require an ACP to support in shaping the future of palliative care in West Cheshire, not only for patients and families but also for our community teams delivering end of life care every day.

We want an experienced practitioner who will bring advanced clinical expertise and an evidence based approach to lead the way for improved integrated community-based specialist palliative care.

**We offer a relocation package of up to £8,000**

The relocation package is for anyone who would need to relocate in order to take up employment with Cheshire and Wirral Partnership.

To be eligible for the relocation payment, you must live more than 40 miles from the base/location of the post you are applying for and you would need to relocate to within 15 miles of the new base.

Main duties of the job

This role will provide team leadership in driving clinical workforce development and embed palliative care competencies across community services. The ACP will be required to develop enhanced clinical skills within the specialist team, facilitating clinical supervision, supporting autonomy and advanced clinical decision making. Immediate key objectives will be to lead on the expansion of non-medical prescribing, empowering specialist clinicians and the wider workforce to deliver timely and effective symptom management. This role will champion patient safety, continuous improvement and collaborative working with system partners.

With support from the specialist team and within the wider care group the ACP will lead on driving quality person centred outcomes, research and innovation. There will need to be a strong focus on clinical audit, service evaluation and continuous quality improvement initiatives.

Qualifications
  • Primary degree
  • Master’s degree in Advanced Practice, Palliative Care or equivalent
  • Non-medical prescriber V300
  • Clinical Examination and Diagnostics modules if not part of Master’s qualifications
  • Teaching qualification
  • Clinical supervisor
  • Community nursing specialist qualification
Knowledge and Expertise
  • Awareness of current national and local agenda in health and social care
  • Demonstration of high level knowledge of multi-agency working (i.e. housing, social services, independent sector, etc)
  • Knowledge and understanding of clinical governance.
  • Quality improvement and change management
  • Demonstrate the ability to work in a highly demanding environment
  • Teaching and mentorship clinical setting
  • Ability to work independently and make autonomous clinical decision within own area of competence
