Job summary

Locum Ready is seeking an Advanced Clinical Practitioner to support regional community health and primary care services across Glynneath. This role offers flexible sessional work within community clinics, ideal for ACPs with autonomous clinical assessment and urgent patient demand experience.

You will work with established clinical teams, delivering advanced decision-making, routine assessments and urgent care support across structured service environments.

Qualifications

• Advanced Clinical Practitioner with Level 7 qualification.
• Background in nursing, paramedic science, AHP or equivalent.
• Independent Prescribing (V300) desirable.

Responsibilities

• Deliver ACP-led sessions in community and primary care settings.
• Manage acute, undifferentiated and minor illness presentations.
• Support urgent patient demand via telephone or face-to-face triage.
• Prescribe safely within scope of competence.
• Record accurate and clear digital documentation.
• Manage clinical actions including results, medication reviews and follow-up tasks.

• Managing acute, undifferentiated and minor illness presentations
• Supporting urgent patient demand through telephone or face-to-face triage
• Prescribing safely within scope of competence
• Managing clinical actions including results, medication reviews and follow-up tasks
• Recording accurate and clear digital documentation

Requirements

• Advanced Clinical Practitioner qualification (Level 7)
• Clinical background in Nursing, Paramedic Science, AHP or similar
• Independent Prescribing (V300) desirable
• Experience in GP, urgent care or community health services
• Ability to work independently in varied clinical environments
• Enhanced DBS
