Locum Ready is seeking an Advanced Clinical Practitioner to support regional community health and primary care services across Greencroft Wynd. Qualifications

• Qualified ACP with autonomous clinical assessment experience.
• Clinical background in nursing, paramedic science, AHP or similar.
• Experience in GP, urgent care or community health services.
• Ability to work independently in varied clinical environments.
• Enhanced DBS clearance.

Responsibilities

• Deliver ACP-led sessions within community and primary care settings.
• Manage acute, undifferentiated and minor illness presentations.
• Support urgent patient demand through triage by telephone or in person.
• Prescribe safely within scope of competence.
• Manage clinical actions including results, medication reviews and follow-up tasks.
• Record accurate and clear digital documentation.

Requirements

• Advanced Clinical Practitioner qualification (Level 7)
