Job summary

HCRG Care Group Holdings Limited is seeking an Advanced Clinical Practitioner (Band 8a) for Bath, delivering autonomous, expert clinical care within community hospital services. You will work across Bath and Wiltshire, supporting complex, frailty-related conditions while promoting audits and service improvements.

The role offers part-time or full-time flexible working hours with a Monday–Friday 8am–4pm pattern and an emphasis on clinical leadership and quality improvement.

Qualifications

• Must be an autonomous healthcare professional with MSc in Advanced Clinical Practice.
• Independent Prescriber qualification and post-registration clinical experience required.
• Willingness to work flexibly across Bath, Chippenham, Swindon and Wiltshire.

We are seeking enthusiastic and ambitious Advanced Clinical Practitioners to join our growing community hospital AP workforce across Bath and North East Somerset, Swindon and Wiltshire. As an Advanced Clinical Practitioner, you will deliver autonomous, expert clinical care within community hospital services, working across organisational boundaries to support patients with complex and frailty-related conditions. We offer part-time or full-time flexible working hours Hours: Monday - Friday 8am - 4pm.

Main Responsibilities Deliver autonomous advanced clinical assessment, diagnosis and treatment. Manage patients with complex, frailty-related and deteriorating conditions. Coordinate patient care across organisational and professional boundaries. Support admission avoidance and timely discharge planning. Lead and contribute to clinical audits and service improvements. Promote evidence-based practice and continuous quality improvement.

The Ideal Candidate Essential Registered healthcare professional MSc Advanced Clinical Practice Independent Prescriber qualification Significant post-registration clinical experience Current full driving license and use of a car during working hours. Desirable Experience of working within community hospital, rehabilitation or frailty services Evidence of audit, quality improvement or service development activity Experience of supporting or mentoring junior staff or students Membership of a relevant professional network or special interest group Other requirements: Willingness to work flexibly across Bath, Chippenham, Swindon and Wiltshire.
