Newton & Haydock Primary Care Network: About Us
Newton and Haydock Primary Care Network consists of 4 NHS GP practices in Newton-le-Willows and Haydock, across several sites. The Network enables NHS staff to work across the practices to help deliver services to 50,000 patients.

The Role
The Clinical Lead for Frailty will be responsible for leading and developing a person-centred PCN Frailty Team pathway across Haydock and Newton PCN.
The post holder will be operating at a strategic level to compliment, influence and collaborate with the Clinical director and all stakeholders, in the development and delivery of effective care to older adults with frailty. The post holder will spend minimum 50% of their time in direct autonomous clinical practice, with clinical responsibility for a defined cohort of patients and their carer's and will provide leadership for clinicians within this service area and 50% of their time operationally managing the PCN Frailty Team.
The Frailty Clinical lead will be integral to the delivery of safe, effective, consistent, and high-quality care for older patients and their carer's. They will be instrumental in promoting clinical excellence and best practice through audits, identification of research opportunities, dissemination of research findings, utilisation of national policy and collaboration with colleagues.
Responsibilities Under four key dimensions

Advanced and expert level practice (50%)
· Autonomously assessing and treating patients; ordering and interpretation of investigations and non-medical prescribing; demonstrating complex reasoning, critical thinking and independent decision making to manage the complex needs of this client group.
· Develop and maintain expertise and appropriate competences in highly developed skills, ensuring the highest level of precision to maintain patient safety in line with Trust and NMC regulatory frameworks.
· Acting within own professional limits and in line with local and national policies.
· Prioritise and manage the complex patients referred for an urgent community response and or proactive frailty assessment working closely alongside the ANPs/specialist practitioners, Geriatricians, and wider MDT, within the parameters of the commissioned and funded services.
· Refer patients, as appropriate, to secondary care and for appropriate diagnostic investigations
· Refer patients to other specialist services or colleagues within or outside the organisation based upon own clinical assessment and judgement.
· Engage patients, families and carers in the care being planned and provided to promote individualised care, with consideration of the available community provision and ensuring the information is realistic, consistent and appropriate for them.
· To be mindful of patients' needs, wishes and culture encouraging self-management. Champion patients' rights, for example, dignity, equality, diversity choice and respect. Apply varied and situationally appropriate methods of communications to promote accurate information sharing.
· Allocate, coordinate, monitor and assess own workload and that of colleagues or the service making adjustments to ensure consistent service delivery.
· Facilitation and active participation in MDT meetings within the service and externally with GPs
· Apply advanced theoretical, evidence based and practical knowledge to the clinical care of older people living with frailty when delivering and shaping services
· Ensures own ongoing professional development both clinically and academically, identifying a close supervision resource and being receptive to professional guidance from consultant colleagues, ensuring compliance with NMC revalidation requirements.
Consultancy and Leadership (50%)
· To lead on the development of a frailty model of care within the Primary Care Network. They shall represent and champion the needs of older adults living with frailty and inform a PCN wide policy that incorporates frailty assessment and ageing well strategies.
· To act as a source of clinical and nursing expertise in the care pathways & care of older adults living with frailty, to all groups of professionals, championing and promoting awareness of frailty and the needs of older adults across the PCN.
· Identifying local, demographic, cultural and environmental factors that influence the health and quality of life for this patient group to influence the commissioning and development of PCN frailty services through systems leadership in both acute and primary sectors.
· Provide mentorship and clinical support to aspiring practitioners acting as role model, educator, and coach for the existing service and as it develops, ensuring sustainability through succession planning within the team, identifying talent and skills within the team.
· Demonstrate compassionate leadership and champion the importance of staff health and wellbeing.
· Attendance at key meetings as required.
· Represent senior colleagues as required.
· Ensure financial targets are met through effective use of resources.
· Identify any financial risks relating to provided services and report back to the Service Manager
· Maximise the use of resources, ensuring high quality care.
Development and provision of education and training
· Ensure the clinical environment is conducive to supporting the education and learning of all staff and students.
· In collaboration with the training hub and other education providers formulate a clear development pathway and skills portfolio for the MDT team working within the PCN Frailty team, to ensure consistent future service provision, and the application of safe, contemporary, evidenced based autonomous practice.
· Through existing cross professional education programmes and through the development of new teaching packages, increase PCN wide knowledge of frailty and associated syndromes increasing practitioner confidence and patient experience
· Develop partnerships with local higher education institutions for the purpose of robust education programmes and potential research opportunities to build quality, creativity and innovation into the safe delivery of the PCN frailty services.
Service development, evaluation, research and audit
· Develop and implement protocols, internal professional and clinical standards, policies and clinical guidance related to the PCN Frailty pathway that is in line with national and local guidance and strategies.
· Developing close relationships with the wider MDT, and alongside the Allied Health Professionals Lead on existing work, develop cross competency assessments and working within the PCN service.
· Involvement in peer reviews of own practice and that of others through internal and external sources.
· Provide assurance to Haydock and Newton PCN, that care is being delivered to the standards set by the PCN through corporate reporting metrics and that the outcomes are shared within the team ensuring they understand their role in addressing any deficits and that poor performance is addressed as per PCN policy.
· Accountable for the quality governance within the identified areas of practice, ensuring the prompt resolution of incidents, investigations and complaints related to the PCN Service.
· Actively participate in and contribute to national and local frailty networks to ensure on-going development and refinement of services, research, education and practice ensuring services continue to stay in line with national recommendations and continue to meet the needs of the local population.
HR
Staff Leadership and Management
· Line management of the MDT Team under the PCN Frailty service, acting as role model, mentor, educator, coach and clinical support for the existing and new staff and as it develops, ensuring sustainability through succession planning within the team, identifying talent and skills within the team.
· To follow performance management policy as required.
· To recognise and celebrate achievements of staff and share throughout the organisation.
· Promote effective communications and relationships within the service sharing good practice and innovation.
Network Policies and Procedures:
The post holder is required to comply with Network Policies, Procedures and Standards at all times.
Confidentiality:
The post holder is required to maintain the confidentiality of information regarding patients, staff
and other health service business in accordance with the Caldicott Guidelines and Data Protection
Act and Children's Act and all other relevant legislation as appropriate.
Risk Management:
All staff have a responsibility to report any risks and clinical and non-clinical accidents and incidents
promptly and co-operate with any investigations undertaken.
Health and Safety:
All staff must be aware of their responsibilities under the Health and Safety at Work Act 1974 and
must ensure that the agreed safety procedures are carried out to maintain a safe environment for
employees, patients and visitors to the Trust.

Appraisal and Statutory Training:
All newly appointed staff will receive an initial appraisal within a months of commencing in the post.
Thereafter, appraisals will be conducted in line with the requirements of the probationary period then annually.
The post holder will undertake all statutory and mandatory training as deemed necessary by the
Trust.
Safeguarding Statement:
All staff will receive appropriate training and induction so that they understand their roles and
responsibilities and are confident about carrying them out.
