Advanced Clinical Practitioner (ACP) Clinical Lead

We are currently recruiting an Advanced Clinical Practitioner Clinical Lead to join one of our practices on the East Sussex/Kent border. Main duties of the job

Main Role and Responsibilities:

  • Provide advanced clinical practice, using expert knowledge and skills to deliver holistic primary care.
  • Assess, diagnose, and treat patients with a range of acute, non-acute, and chronic conditions.
  • Refer patients to multidisciplinary team members, secondary care, and other organisations as appropriate.
  • Perform and interpret diagnostic tests and routine procedures such as cytology, venepuncture, and blood pressure monitoring.
  • Support and advise colleagues where protocols do not apply.
  • Communicate sensitive information effectively to patients, families, and carers.
  • Initiate diagnosis processes for chronic diseases like Diabetes, COPD, asthma, and IHD, referring to specialists as needed.
  • Contribute to practice targets, including QOF, prescribing schemes, and benchmarking.
  • Audit care outcomes and implement improvements.
  • Line manage the ANP and nursing team.
  • Adhere to professional guidelines and practice policies, including infection control and risk management.
  • Possess extended and supplementary prescribing qualifications.

Job responsibilities

Person Specification
Qualifications
  • Level 7 (Masters) qualification in Advanced Clinical Practice or equivalent, with a recognised healthcare/professional registration.
Experience
  • Experience working in a primary care setting.
