Job summary

HCRG Care Group is seeking an experienced Advanced Clinical Practitioner to join the Rapid Response Service covering the Swale locality. The post involves autonomous clinical assessment, diagnosis, and care planning for urgent needs, working with community nurses, GPs, and system partners to prevent hospital admissions.

The role requires independent prescribing, NMC registration, and CPD evidence, with a 37.5-hour week across a seven-day pattern.

Qualifications

• Current registration with the Nursing and Midwifery Council (NMC).
• Advanced Clinical Practitioner qualification or equivalent advanced practice training.
• Independent Prescribing qualification.
• Evidence of continuing professional development and education at degree level or above.
• Significant post-registration experience within urgent care, community services, primary care, emergency care or related clinical setting.
• Proven experience of advanced clinical assessment and decision-making.

Responsibilities

• Undertake advanced clinical assessments of patients presenting with urgent, acute and complex healthcare needs in community settings.
• Work autonomously to assess, diagnose, treat and manage patients within your scope of practice.
• Prescribe and review medications in line with independent prescribing qualifications and best practice guidelines.
• Develop and implement patient-centred care and treatment plans to support admission avoidance and early intervention.
• Request, interpret and act upon appropriate diagnostic investigations.
• Provide clinical expertise and support to members of the multidisciplinary team.
• Work collaboratively with system partners to ensure seamless patient care and effective discharge and admission avoidance pathways.
• Recognise and respond appropriately to deteriorating patients, escalating concerns where necessary.
• Maintain accurate and contemporaneous clinical records in line with professional and organisational standards.
• Participate in clinical audit, governance, service development and quality improvement initiatives.
• Act as a role model, supporting the development of colleagues and promoting high standards of clinical practice.

Job Introduction

We have an exciting opportunity for an experienced and motivated Advanced Clinical Practitioner to join our Rapid Response Service covering the Swale locality.

The Rapid Response Service provides urgent assessment and intervention for patients experiencing acute health deterioration, helping to prevent unnecessary hospital admissions and supporting people to remain safely in their own homes. As an ACP, you will work as an autonomous practitioner, delivering advanced clinical assessment, diagnosis, treatment and care planning for patients with a wide range of urgent, acute and complex healthcare needs.

Working closely with community nurses, therapists, GPs, ambulance services, acute trusts and wider system partners, you will play a key role in delivering high-quality, responsive care across the Integrated Care System (ICS).

This is a full-time position working 37.5 hours per week across a seven-day service, with flexibility to meet the needs of the service.

Main Responsibilities

The Ideal Candidate

The ideal candidate will have:

Essential

• Current registration with the Nursing and Midwifery Council (NMC).
• Advanced Clinical Practitioner qualification or equivalent advanced practice training.
• Independent Prescribing qualification.
• Evidence of continuing professional development and education at degree level or above.
• Significant post-registration experience within urgent care, community services, primary care, emergency care or a related clinical setting.
• Proven experience of advanced clinical assessment and decision-making.
• Experience managing patients with acute, chronic and complex conditions.
• Ability to work autonomously and manage clinical risk effectively.
• Excellent communication, leadership and organisational skills.
• Strong IT skills, including the use of clinical systems and Microsoft Office applications.
• Full UK driving licence and access to a vehicle for work purposes.

Desirable

• Experience working within an Urgent Community Response, Rapid Response, Virtual Ward or Hospital at Home service.
• Teaching, mentorship or assessor qualification.
• Experience supporting junior staff, students and multidisciplinary colleagues.
• Experience participating in service improvement, audit and governance activities.
