This Advanced Clinical Practitioner role will involveworking alongside our multi-disciplinary team which consists of a GP Partners,Advanced Clinical Practitioner Partner and Pharmacist Partner. They are allsupported by Advanced Clinical Practitioners who lead on acute same day care,alongside the Home Visiting Team, First Contact Physio, Pharmacists,Practice Nurses and HCAs. We also have Social Prescribers who work as PCNstaff dealing with the Mental Health of our patients.
The successful Advanced Clinical Practitioner willwork as an autonomous practitioner, diagnosing, managing, prescribing anddischarging patients with illness or injury across all age groups includingyoung children. This may be by face-to-face consultation, telephone or videoconsultation.

Main duties of the jobThe Advanced Clinical Practitioner (ACP) has been assessed as competent in practice using theirexpert clinical knowledge and skills. They have the freedom and authority toact and make autonomous decisions in the assessment, diagnosis and treatment ofpatients.
The ACPprovides an opportunity for patients to avoid the need to be referred to a GP,thereby enabling them to receive timely care and negate unnecessary delay inreceiving treatment.
ACPs in primarycare can develop close, long-term relationships with their patients and work inpartnership with them to achieve optimum health. They are autonomous in makingdecisions based on assessment, diagnosis and interpretation of test results. Theycan independently prescribe appropriate medication, evaluate or refer to otherspecialists if necessary.
