Main duties of the job

If you are passionate about making a difference in the lives of older individuals living with frailty then we're on the lookout for an experienced, dedicated professional to join our vibrant team in the innovative SDEC unit, where compassion meets cutting-edge care.

In this role, you'll play a pivotal part in assessing and managing care for our cherished older population. Your mission? To ensure every patient receives tailored, high-quality care in the safest and most suitable setting. We believe in the importance of keeping our patients out of the hospital whenever possible. Person Specification
Qualifications
  • Appropriate qualifications and registration
  • Geriatric medicine module within the ACP qualification
  • Registered Nurse with current NMC registration (Part 1) or AHP with current HCPC registration
Experience
  • Experience with working at a senior level
  • Experience with working with a wider MDT
  • Substantial years experience working with frail patients
