Job overview

The service requires a fully qualified Advanced Clinical Practitioner to support the urgent and emergency needs of the patients at Rampton Hospital. A high level of clinical expertise is required to enhance patient care: ACP MSci or equivalent, V300 Prescribing, Minor Illness/Minor Injury Management.

Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced), this cost will be deducted from your salary over the first 2 months of employment.

You are encouraged to enrol for the DBS Update Service. An annual fee of £16 per year applies.

To demonstrate an in depth knowledge and capabilities in all aspects of health and social care encompassing advanced clinical assessment, critical thinking and clinical management skills that are evidence based and deliver high quality patient centred care. To demonstrate complex decision-making skills and competences for expanded scope of advanced clinical practice for service users and clinical teams according to needs.

Detailed job description and main responsibilities

The ACP is accountable for independent clinical assessment, formulation, diagnosis and treatment of these individuals with undiagnosed and undifferentiated conditions. They will provide expert professional and independent clinical care which enables the coordination of a multi-professional seamless service. Practice in compliance with respective code of professional conduct and within scope of practice, being responsible and accountable for their decisions, actions and omissions.

Demonstrate a critical understanding of their broadened level of responsibility and autonomy and the limits of own competence and professional scope of practice, including when working with complexity, risk, uncertainty and incomplete information.

Act on professional judgement about when to seek help, demonstrating critical reflection on own practice, self-awareness, emotional intelligence, and openness to change.

Work in partnership with individuals, families and carers, using a range of assessment methods (e.g. of history-taking; holistic assessment; identifying risk factors; mental health assessments; requesting, undertaking and/or interpreting diagnostic tests and conducting health needs assessments).

Demonstrate effective communication skills supporting people in making decisions, planning care or seeking to make positive changes, through person-centred approaches in health and care. se expertise and decision-making skills to inform clinical reasoning approaches when dealing with differentiated and undifferentiated individual presentations and complex situations, synthesising information from multiple sources to make appropriate, evidence-based judgements and/or diagnoses.

Initiate, evaluate and modify a range of interventions which may include prescribing medicines, therapies, life style advice and care. Exercise professional judgement to manage risk appropriately, especially where there may be complex and unpredictable events and supporting teams to do likewise to ensure safety of individuals, families and carers. Work collaboratively with an appropriate range of multi-agency and inter-professional resources, developing, maintaining and evaluating links to manage risk and issues across organisations and settings.

Act as a clinical role model/advocate for developing and delivering care that is responsive to changing requirements, informed by an understanding of local population health needs, agencies and networks.

Evidence the underpinning subject-specific competencies i.e. knowledge, skills and behaviours relevant to the role setting and scope, and demonstrate application of the capabilities to these, in an approach that is appropriate to the individual role, setting and scope. Maintain clinical skills and in confidence in minor surgery skills for surgical closure of wound and removal of foreign bodies.
