Job description

New Islington Medical Practice is looking to expand our clinical offering to our ever growing patient population with another ACP to join our robust multi-disciplinary team.

As one of the best rated practices within Greater Manchester, with excellent feedback from patients via the 2025 Patient survey and Google, we continually look to forward plan our team so that we can continue to maintain compassionate and high quality care to our patients, whilst prioritising the well-being of our staff.

We are looking for another ACP to join our clinical workforce where you can look forward to working in a supportive environment within which your advanced clinical skills and development ambitions will be valued and nurtured. You will work alongside our existing ACP, and help manage the daily acute workload servicing the needs of our 12500 patients.

You will deliver high quality, person centred care in a variety of settings, undertaking clinical face to face sessions and home visits, alongside remote telephone consultations. You will be working autonomously, drawing on your own skills whilst being supported and being able to collaborate closely with our experienced multi-professional team.

Main duties of the job

Clinically, your focus will be on delivering evidence-based practice to patients with a variety of presentations, utilising your skills to assess, diagnose and treat patients with both acute and chronic conditions.

As an autonomous practitioner, you will be responsible for the care delivered and will need to demonstrate critical thinking and clinical decision-making skills.

You will conduct both face-to-face and telephone triage appointments and work dynamically and collaboratively to meet the patients' daily needs. You will participate in occasional home visits when clinically necessary and share workload with our existing ACP to lead care for patients with learning disabilities.

You should be able to manage clinical episodes independently, including prescribing and onward referrals, and be able to review test results resulting from your own work whilst also participating in managing the overall workload generated by other clinicians.

We also encourage professional development and can offer internal and external education to help you explore your specialist interest.

New Islington Medical Practice is a large well established practice serving the needs of over 12500 patients and we are passionate about delivering high quality care with an industry leading same day service that we are incredibly proud to offer.

We are a multi disciplinary team made up of a variety of clinicians made up of GPs, an existing ACP, practice nurses, HCA, phlebotomist, training GPST and FY2 doctors, physiotherapists, mental health nurse and practice based pharmacists. Our admin and reception team round things off, with 4 managers working alongside our GP partners.

We're a close knit team made up of staff taken all the way from their apprentice days into fully trained positions and also staff members who have joined us in the past year.

Working in the vibrant City Centre, our patient population is growing daily and we have quite a mix of patient needs.

Job responsibilities
  • Use highly developed clinical knowledge to independently assess, diagnose, plan, implement and evaluate treatments and interventions for patients presenting with complex undifferentiated or undefined presentations.
  • Undertake physical examination as indicated by the patients condition; autonomously analysing complex clinical signs and investigation results to consider differential diagnoses and diagnose conditions, initiating treatment as required.
  • If qualified to do so, autonomously undertake assessment of children who present with an illness or injury; to determine a clinical diagnosis, treatment and appropriate discharge within your professional boundaries and in line with training, skills and competencies.
  • Request investigations such as blood, urine and other laboratory tests, electrocardiographs (ECGs), ultrasound scans, X-Rays and computed tomography (CT) scans in accordance with IR(ME)R regulations.
  • Analyse multiple sources of data including patient history, physical examination and investigation findings when making diagnoses, clinical judgements and evaluating care provided and liaising with GPs for advice when scope of practice is exceeded.
  • Autonomously formulate appropriate management plans for patients, formulating clinical decisions and often complex treatment plans to manage acute illness and acute episodes of chronic illness including referral, admission, or discharge from service.
  • Prioritise health problems and intervene appropriately in complex, urgent and emergency situations, including initiation and leadership of resuscitation.
  • Provide clinical cover to the surgery according to patient and staffing needs and contribute to the On call team optionally as an additional element with enhanced pay offered for this level of clinical leadership.
  • Prescribe and review medication for therapeutic effectiveness appropriate to patient need and in accordance with best/evidence-based practice and national and local protocols
  • Communicate effectively with patients, carers, colleagues, and others using appropriate communication styles. Anticipate barriers to communication and ensure patients and significant others are kept fully informed and consent to treatment.
Person Specification
Experience
  • Experience in working clinically in an advanced practice role
  • Have an up to date portofolio of supervised practice in advanced clinical practice
  • Experience of dealing with vulnerable patients
  • Experience of working within the primary care sector
  • Experience of utilising clinical systems such as EMIS, Docman and online consultation platforms
Qualifications
  • Registration with the Nursing and Midwifery Council (NMC) or Health and Care Professions Council
  • Advanced Nurse Practitioner (RCN Accredited) Or Advanced Clinical Practitioner (RCEM Accredited) or equivalent qualification.
  • Experience in General Practice/ Walk-in Centre/ Urgent Care Centre/ Emergency Department/Out of Hours Service desirable
  • Evidence of undertaking clinical audits
  • Up to date knowledge of DOH and NICE guidelines
  • Be able to discuss in depth current clinical and managerial issues regarding safeguarding adults/children and domestic abuse
  • Experience with utilising EMIS as a clinical system
