A leading healthcare provider in London seeks an experienced Advanced Clinical Practitioner to join the Frailty Virtual Ward team. This role involves assessing patients, developing individualized care plans, and providing management of medical conditions to enhance patients' quality of life at home. Key responsibilities include collaboration with general practitioners, using digital tools for monitoring, and leading a clinical team.
We are looking to recruit an experienced and highly motivated advanced clinical practitioner, either a nurse or allied health professional (AHP), to join the Frailty Virtual Ward team. The role will primarily involve working closely with the clinical lead, liaising with general practitioners and community teams to assess patients who meet the criteria for the frailty virtual ward. The post holder will contribute to the development of individualized care plans and provide expert management of medical conditions, ensuring patients receive high-quality, personalized care. Additionally, the post holder will apply their expertise to safely manage patients in the comfort of their own homes, helping to improve outcomes and enhance the quality of life for those with frailty.
