An established healthcare provider is seeking a dedicated advanced clinical practitioner to join their innovative Frailty Virtual Ward team. We are looking to recruit an experienced and highly motivated advanced clinical practitioner, either a nurse or allied health professional (AHP), to join the Frailty Virtual Ward team. The role will primarily involve working closely with the clinical lead, liaising with general practitioners and community teams to assess patients who meet the criteria for the frailty virtual ward. The post holder will contribute to the development of individualized care plans and provide expert management of medical conditions, ensuring patients receive high-quality, personalized care. Additionally, the post holder will apply their expertise to safely manage patients in the comfort of their own homes, helping to improve outcomes and enhance the quality of life for those with frailty.
