Job description

We are seeking an experienced and dynamic Advanced Clinical Practitioner (ACP) to join our multidisciplinary Enhanced Primary Care Mental Health Service.

The post holder will play a key role in delivering high-quality, patient-centred care by providing comprehensive physical health assessments for individuals with long-term health conditions and mental health conditions, and conducting annual physical health checks for patients under shared care arrangements.

The ACP will work autonomously and as part of a team to support early identification of physical health issues, provide ongoing management of co-existing physical and mental health conditions, and ensure equitable access to healthcare for this vulnerable population.

The post holder must be a qualified independent prescriber and will utilise their advanced clinical skills to diagnose, manage, and prescribe as appropriate. The ACP will be clinically responsible to prescribe in the absence of a GP.
