Ombersley Medical Centre are looking for an Advanced Clinical Practitioner (ACP) to join our dynamic and forward thinking team in beautiful rural Worcestershire.
We aim to provide high quality, personalized, patient centered care in a safe and effective setting. We ensure patients are involved at all points of their journey and strive to improve the health status of those we care for.
The ACP is a new role to our practice and will work alongside the Duty Doctor caring for patients who present with acute illnesses as well as some chronic disease management in a mix of face to face and telephone appointments. The team at Ombersley Medical Centre are encouraged to develop and progress, with supportive, organised and innovative thinking for our team as well as our patients.
The practice team hold regular meetings with opportunities for learning and discussion, deliver a wide MDT approach to patient care and offer a variety of enhanced services to our community.

Main duties of the job
The role of the Advanced Clinical Practitioner(ACP) will have the freedom and authority to act and make autonomous decisions in the assessment, diagnosis and treatment of patients.
The role provides an opportunity for patients to avoid the need to be referred to a GP, thereby enabling them to receive timely care and negate unnecessary delay in receiving treatment for on the day, acute issues, from initial history taking, clinical assessment, diagnosis, treatment, prescribing and evaluation of care. To include acute, minor and long term conditions in line with existing practice and national policies and guidelines.
The ACP will be autonomous in making decisions based on assessment, diagnosis and interpretation of test results and will initiate, evaluate and modify a range of interventions which may include prescribing medicines, therapies, lifestyle advice and care.
