Main duties of the job

If you are passionate about making a difference in the lives of older individuals living with frailty then we're on the lookout for a dedicated professional to join our vibrant team in the innovative SDEC unit, where compassion meets cutting-edge care!

In this role, you'll play a pivotal part in assessing and managing care for our cherished older population. Your mission? To ensure every patient receives tailored, high-quality care in the safest and most suitable setting. We believe in the importance of keeping our patients out of the hospital whenever possible.
