  • Autonomous assessment, diagnosis, treatment and management of patients with acute and chronic presentations
  • Lead specialist management of frailty patients including comprehensive geriatric assessments
  • Advanced cardiovascular disease management including risk stratification, medication optimization, and care coordination
  • Demonstrate expert-level clinical decision making within the primary care urgent care setting
  • Develop and deliver specialist pathways for complex multimorbidity management
  • Manage complex, undifferentiated and undiagnosed conditions with confidence
  • Provide high-quality, patient-centred healthcare services with focus on vulnerable populations
Team Leadership & Collaboration
  • Work as an integral part of our Advanced Clinical Practitioner-led urgent care team
  • Collaborate seamlessly with multidisciplinary healthcare professionals
