Job description

The Advanced Clinical Practitioner (ACP) will work autonomously in a primary care setting, using their advanced clinical skills to assess, diagnose, treat, and manage patients with a wide range of health conditions. The ACP will support the delivery of high-quality, patient-centered care and contribute to service development and the training of staff.

Main duties of the job

Key Responsibilities:

Clinical Practice

Undertake autonomous consultations, including assessment, diagnosis, treatment, and evaluation.

Manage both acute and chronic conditions, including minor illness and injury.

Prescribe medications within scope of practice and according to local and national guidelines.

Request and interpret diagnostic tests (e.g. blood tests, imaging).

Make appropriate referrals to secondary or specialist services.

Provide health promotion and disease prevention advice.

Seven Dials Medical Centre proudly serves just over 7,000patients in Brighton and Hove and has 14 members of staff including 3 GPPartners, an ACP, Practice Nurse and HCA. We work closely with the PCN to offerpatients access to Social Prescribers, Mental Health Co-ordinators, CareCo-ordinators and a Pharmacy team.

Job responsibilities

Clinical Practice

Contribute to the strategic development of clinical services.

Participate in clinical audits and service improvement projects.

Act as a clinical role model and mentor to other staff.

Work collaboratively with the multi-disciplinary team (MDT).

Education and Training

Support the education and professional development of junior staff.

Participate in CPD activities and mandatory training.

Maintain clinical competence through reflective practice and appraisal.

Person Specification
Qualifications
  • - Registered healthcare professional (e.g. RN, Paramedic, Physiotherapist)
  • - MSc in Advanced Clinical Practice or equivalent
  • - Independent Non-Medical Prescriber (V300)
  • Excellent clinical reasoning and decision-making skills
  • Ability to work independently and as part of a team
  • Strong communication and interpersonal skills
  • IT proficiency and familiarity with clinical systems (e.g. SystemOne, Emis)
Experience
  • - Proven autonomous practice in patient assessment and management
  • - Experience in leadership or supervisory roles an advantage
  • - Experience in a primary care setting desirable
