A leading healthcare provider in Bolton is looking for an experienced Nurse or AHP with a Master's in Advanced Clinical Practice. This permanent role focuses on delivering urgent community responses and virtual wards, requiring excellent communication and organisational skills. The candidate must be able to work autonomously and travel independently across the locality. Competitive training and development opportunities are available.
We are seeking to appoint a dynamic, confident, innovative and flexible Nurse or AHP with excellent communication and organisational skills. The post is 37.5 hours per week and is a permanent position to support the delivery of urgent community response and hospital at home/virtual wards.
This post holder will join our existing team of high functioning, highly motivated Advanced Clinical Practitioners, who deliver a wide variety of services including 2 hour urgent community response, hospital at home/virtual ward, falls pick up services and dedicated care home support, all enable people to be cared for in their usual place of residence. The Advanced Clinical Practitioners operate seven days a week between 08.00 to 20.00.
The post holder will work with partners system wide to provide integrated care across the health economy. The applicant will have qualified and hold an MSC in Advanced Clinical Practice.
The applicant must be able to travel across the locality independently in accordance with the Equality Act 2010.
