Title: Advanced Clinical Practitioner Complex Lives
Hours: 16 hours per week
Service Operates: Monday to Friday between 0800-1830
Pay: £27.49 - £30.63 depending on qualifications and experience
Reports to: Clinical Manager
Accountable to: Head of Nursing and Clinical Workforce
Base: Newfield House, Blackpool - undertaking Clinics across Blackpool, Fylde and Wyre
Job Summary

Duties And Responsibilities
- Practice autonomously, as an independent practitioner/prescriber, participating in the care of and undertaking assessment, diagnosis, treatment / referral, and discharge of patients presenting with minor illness or injury within agreed parameters
- Support the Management team to ensure all QUOF Targets are met
- Provide support for junior members of staff
- Working closely with the management team to develop and support new opportunities
- Provide advanced clinical skills and advice demonstrating a sound understanding of the issues related to identification, assessment, diagnosis, treatment, and management of a client group whilst working autonomously
- Identify the need for and initiate and interpret relevant diagnostic investigations and results
- To safely and competently undertake invasive and non-invasive procedures to establish a diagnosis
- To work within the parameters of current nurse prescribing legislation
- Monitor and lead improvements to standards of care through supervision of practice, clinical audit, and evidence-based practice, teaching and supporting professional colleagues and the provision of skilled professional leadership
- Act in accordance with the NMC Code of Professional Conduct
- Provide professional and clinical advice within the multi-disciplinary team
- Produce accurate, contemporaneous, and complete records of patient consultation, consistent with legislation, policies and procedures
- Prioritise, organise, and manage own workload in a manner that maintains and promotes quality
- Deliver care according to local and national guidelines
- Obtain and document consent for any treatment undertaken and keep the patient at the centre of his/her own care, ensuring full participation in decision making
- Work within all relevant policies and procedural guidelines e.g., infection control, chaperoning and risk management
- Safeguarding responsibilities for adults and vulnerable groups, reporting concerns appropriately at the correct time and engaging with services as needed.
- The Head of nursing may also ask for other duties to be carried out to the benefit of the organisation.
Clinical practice
- Autonomously assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis and those with long term conditions in line with national and local policies and practice/ network guidelines
- Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan
- Prescribe medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice
- Work with patients to support concordance with and adherence to prescribed treatments, provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects, and interactions
- Promote health and well-being using health promotion, health education, screening, and therapeutic communication skills
- Communicates highly sensitive condition related information to patients, relatives, and carers
- To take a lead in mental health reviews and assessment of patients including working alongside the pharmacist to review mental health medication
- When required, ensure patients are referred to other members of the primary health care team e.g., podiatry, or secondary care as necessary
- Clinically examine and assess patients needs from a psychological and physiological perspective and plan clinical care accordingly
- Ensure the correct use of clinical templates, read codes and protocols
- Inform the Clinical Manager / Service Coordinator of any untoward incidents e.g., falls, verbal aggression, criminal damage, safeguarding concerns

Role of the Service
Our Complex Lives Business Unit Provides Healthcare To Those Who Are Particularly Vulnerable Across Blackpool Fylde And Wyre And Beyond. It Is Made Of 3 Areas Of Practice
- Compass Medical Practice this a GP practice for those who are difficult to manage in mainstream practice. Patients are either consulted with via the telephone /video link and when a face-to-face consultation is required a risk assessment is undertaken to see what support the clinician requires. Consultations take place with a patient liaison officer in wither our mobile van or in an agreed location pre booked in advance. The medical practice also provides medical cover for those in a local rehab centre Thornton House. We attend a weekly ward round to ensure all patients have contact with a GP/ACP and a Practice Nurse this has built great relationships with the team there. We are part of the Multi-Disciplinary Team meeting for Thornton House as well as being available for any healthcare needs during our opening hours.
- Homeless Health Team - FCMS is currently undertaking a pilot healthcare service for Homeless People within our local community. Our team of clinicians are co-ordinating the care of the homeless population and offering enhanced support for those who are not accessing mainstream services. The team run clinics around the local town centre, working closely with other agencies that support this community. The team offer a comprehensive and holistic assessment of the individuals needs and work with existing health services to meet these and use every contact opportunity to offer health promotion and improve the health outcomes for these patients. The team offer everything from wound care; immunisations; management of long-term condition advice, to supporting with complex care needs.
- Asylum Seeker Clinics we provide new patient health checks to these patients whilst registering them at a local GP practice at the same time. This takes place in the hotel where they are staying with the aid of interpreters.
