Up to 37.5 hours per week available.

Ouraim is to provide high-quality, patient-centred care in the comfort of theirown homes, reducing hospital admissions and length of stay in hospital andenhancing overall well-being through enhanced care coordination.

As an Advanced Clinical Pharmacist, you will be acornerstone of healthcare delivery and will play a critical role in our newmultidisciplinary team, covering two of the six new frailty virtual wards thatwe have planned for our locality.

Together, you will drive seamlesstransitions of care and ensure outstanding outcomes for our patients.
In this role, you will coordinate clinical pharmacy servicesacross the Virtual Wards, employing a holistic approach to medicinesoptimisation across a variety of healthcare settings. Beyond dailyresponsibilities, you will be at the forefront of innovationleading clinicalaudits, spearheading quality improvement projects, and contributing to innovativeresearch that pushes the boundaries of whats possible in-patient care.

You will also play a key role in shapingthe future of Medicines Optimisation within Pharmacy Services, providingstrategic input while maintaining a strong presence within the organisation.Your passion for excellence in pharmacy will help develop and inspire theVirtual Ward Pharmacy Team, as well as healthcare professionals includingNon-Medical Prescribers (NMPs), ensuring the highest standards in medicinesgovernance and clinical care.
