Job description

At Practice Plus Group, we believe in delivering exceptional care that's accessible, efficient, and patient-focused. In Suffolk and North East Essex, our Clinical Assessment Service plays a vital role in supporting patients across the region - providing timely, expert clinical advice and directing them to the most appropriate care pathway.

As an Advanced Nurse / Clinical Practitioner, you'll be part of a dynamic, multidisciplinary team working within a telephone and digital triage environment. You'll help ensure patients receive the right care, in the right place, at the right time - reducing unnecessary hospital visits and improving outcomes across the system.

Practice Plus Group is one of the UK's leading independent healthcare providers, and we're proud to support the NHS with innovative services that put patients first. We can offer both Self Employed (LTD Company or Sole Trader) or Bank (PAYE) contracts for this role.

Responsibilities:
  • Delivering remote clinical assessments via telephone and digital platforms for patients with urgent and unscheduled care needs.
  • Making safe, evidence-based decisions to direct patients to appropriate services, including self-care, primary care, urgent treatment centres, or emergency services.
  • Applying advanced clinical knowledge to assess undifferentiated presentations and manage risk effectively.
  • Supporting clinical governance, contributing to service development, and mentoring colleagues.
  • Collaborating with GPs, paramedics, mental health professionals, and other clinicians to ensure integrated care across the region.
Qualifications:
  • Registered Nurse with valid NMC registration and MSc in Advanced Clinical Practice or equivalent.
  • Independent Prescriber (V300) qualification.
  • Experience in urgent care, triage, or telephone-based clinical assessment is highly desirable.
  • Over 3 years working autonomously.
  • Ability to see and treat patients of all ages, including under 2 years old.
  • Located within an hour's commute to our site in Ipswich.
  • Excellent clinical reasoning, communication, and digital literacy skills.
  • Ability to work autonomously while contributing to a supportive, team-based culture.
  • A passion for improving patient access and outcomes through innovative care delivery.
